Written verifications under the official seal of the Texas Optometry Board are available upon a written request under the Public Information Act. The fee for each request is $40.00. To order, submit a written request with payment by check payable to “Texas Optometry Board.”
The written request should include your name, license number and the state board (including address) where the verification should be sent. If the state board has a required form, please include the form along with the written request.
At this time, the Board does not have an online application or way to pay with a credit card.
